Supporting People with Chronic Conditions in Decision Making | Participants will practise person-centred communication strategies relevant to people with or without cognitive impairment. |
Person-centred Care Planning | Participants will learn to consider the reasons behind behaviours and why people with dementia or chronic conditions respond in certain ways. |
International Classification of Functioning, Disability and Health (ICF) Framework | Participants will investigate how the International Classification of Functioning, Disability and Health (ICF) framework can be used for describing and organising information on functioning and disability. |
Person-centred Record Keeping | Participants will consider the role of client records in delivering person-centred care and develop an action plan with person-centred strategies relevant to the service, their clients and families. |
Developing a Shared Approach | Participants will explore the principles of partnership working with families and develop interventions to encourage the sharing of biographical information to enable person-centred care. |
The Principles of Person-centred Organisational Culture | Participants will explore how organisational culture develops and how this might support or inhibit person-centred care. |
Supporting Teams | Participants will evaluate how teams may be supported to implement ICF and develop strategies essential for person-centred care. |
Leadership | Participants will learn as a leader on how to develop and communicate person-centred practice and policies, to effectively enact person-centred care at an organisational level. |
Working with Caregivers | Participants will learn how to develop processes that create meaningful engagement for clients and families, as part of the organisation. |
